Methemoglobinemia is a pathological condition in which methemoglobin produced in the body exceeds its physiological limit of <1%. Methemoglobin is an altered state of haemoglobin in which iron is in the oxidized ferric rather than the reduced ferrous form, rendering it incapable for binding and transporting oxygen. We report a case of 10 years old boy who presented with cyanosis and respiratory distress after ingestion of paint thinner. History, clinical cyanosis and chocolate coloured blood with SpO2 value of 94% on pulse oximeter were highly suggestive of methemoglobinemia. Patient responded well to artificial ventilation, methylene blue therapy and exchange transfusion and recovered.
